We seek a highly driven, dedicated, and committed individual who excels in teamwork and possesses outstanding time management, interpersonal, and organizational abilities. The ideal candidate should thrive under pressure, demonstrate meticulous attention to detail, and possess excellent communication skills.
Our department focuses on advancing our medicines optimization and sustainability initiatives. The successful candidate will play a crucial role in helping us meet these objectives by conducting medication histories, engaging with service users and caregivers regarding their medications, facilitating leave and discharge planning, utilizing near-patient dispensing, and collaborating with primary care to ensure a seamless transition post-discharge.

Main Responsibilities
The successful applicant will collaborate with specialty pharmacists and all members of the multidisciplinary team (MDT) to coordinate all aspects of the patient's medication journey, ensuring effective communication for a safe and integrated transfer of care.
Key responsibilities include:
- Assisting clinical teams with the supply, storage, and management of medications within inpatient settings.
- Completing medication history and reconciliation upon patient admission.
- Providing information to patients and caregivers about their medications to support adherence and understanding.
- Working closely with clinical teams to facilitate timely leave and discharge, utilizing near-patient dispensing processes to ensure medications are provided promptly.
- Supporting the transfer of care into primary care, utilizing the DMS service and liaising with other healthcare professionals.
- Participating in the accuracy checking rota within the dispensary.
- Contributing to the training and development of trainees.
- Participating in the weekend and bank holiday rota.
