Key Responsibilities

The primary function of the Health Advisor is to guarantee that Patients and Carers achieve optimal outcomes by delivering the appropriate care at the right time and location.

You will be responsible for ensuring that each patient or carer undergoes a Making Every Contact Count (MECC) assessment. Additionally, you will manage and organize all incoming MECC referrals and facilitate bookings into the relevant services as necessary.

Your role will involve providing a high-quality, patient-centered experience within the Care Coordination Hub, assisting all other departments in delivering exceptional patient and carer experiences. You will offer patient and carer-focused telephone and administrative services that guide and direct them in accessing the suitable services or healthcare professionals.

  • Prior experience in an office setting and familiarity with general office/administrative tasks.

  • Experience with patient appointment booking systems, particularly SystmOne.

Qualifications

  • NVQ Level 3 in Business Administration/Customer Services or equivalent knowledge and experience.
  • Proficiency in word processing to RSA 3 standard or ability to demonstrate this skill level.

Knowledge

  • Comprehensive understanding of various administrative procedures and practices.
  • Familiarity with a range of software applications, including Microsoft Word, Excel, Outlook, and EPR systems.

Desirable

  • Knowledge of SystmOne functionalities and other clinical reporting systems.
