About the Role

We are seeking a dedicated and compassionate Support Practitioner to join our team at Highland Home Carers. As a Support Practitioner, you will play a vital role in supporting individuals with learning disabilities in their own homes, providing 24/7 care and assistance with daily living tasks.

Key Responsibilities
  • Support and Assist individuals with personal care, including bathing, dressing, and medication management.
  • Enable Individuals to participate in a wide range of activities, such as social outings, walks, and visits with family and friends.
  • Work in Partnership with Assistant Service Managers and designated professional staff to develop and implement Personal Plans that meet the individual's physical, social, and emotional needs.
  • Manage Finances and maintain records in accordance with Highland Home Carers' policies and procedures.
  • Provide Budgeting Support to ensure individuals can manage their finances effectively.
  • Maintain Tenancies and report any issues to relevant agencies.
  • Tidy Outdoor Spaces and maintain a safe and clean environment.
  • Respect Confidentiality at all times and explain Highland Home Carers' policies on confidentiality to individuals and colleagues.
  • Submit Reports and timesheets as requested.
  • Be Flexible and provide cover for other Support Practitioners when needed.
  • Participate in Training and attend review meetings as required.
Requirements
  • SVQ Level 2 in Social Care or willingness to work towards this qualification.
  • Excellent Communication Skills to work with individuals, families, and health professionals.
  • Flexible Approach to work and a commitment to person-centred support.
