Position Summary

Band 5 - Community Nurse

We are looking for an enthusiastic and proactive full-time Band 5 Community Nurse to join our Integrated Care team.

The Integrated Care team operates in a progressive and innovative manner, providing community-based services. Our multidisciplinary team includes physiotherapists, occupational therapists, nurses, community matrons, and care coordinators, all collaborating to ensure a comprehensive approach to patient care.

Main Responsibilities

The successful candidate will be a Registered Nurse with experience in the NHS. While community experience is advantageous, it is not mandatory. You will be responsible for assessing, managing, planning, and delivering care to your assigned patients.

You will collaborate with your colleagues and senior nursing staff, demonstrating effective teamwork while also working independently. Supervision of pre-registration students and band 4 nursing assistants will be part of your role.

A commitment to collaborative practice is essential, as you will be liaising with various health and social care professionals and agencies.

Proficiency in computer systems is required, as all patient records are maintained electronically.

Working Environment

Probationary Period

This position is subject to a probationary period, with internal applicants exempt unless currently undergoing a probationary period.

High Cost Area Supplement

This role includes a High Cost Area Supplement of 5% of the basic salary.
