About the Role

We are seeking an experienced and motivated Occupational Therapist to join our team at NELFT North East London Foundation Trust. As a key member of our Integrated Community Team, you will play a vital role in providing high-quality patient-centred care and contributing to service development.

Key Responsibilities
  • Work closely with community nurses, community matrons/care coordinators, physiotherapists, occupational therapists, and rehabilitation assistants to maintain independence, promote self-care, and assist patients in achieving their goals.
  • Develop and implement effective rehabilitation plans to support patients with physical conditions, including neurological, orthopaedic, falls, long-term conditions, and musculoskeletal issues.
  • Collaborate with the management team to ensure seamless service delivery and respond to changing service needs.
  • Contribute to the development of new services and initiatives to improve patient outcomes and experience.
Requirements
  • Experience of working at B5/B6 level
  • Previous NHS experience
  • Degree in Occupational Therapy or equivalent
  • HCPC registration
  • Desirable: community rehabilitation, ordering and assessment of equipment needs, supervision of junior staff/student, experience of multi-professional team working
