Qualifications
Required Qualifications
- A Diploma Level 2 in Health and Social Care or an equivalent qualification is preferred.
Required Experience
- Proven experience in healthcare or community services, particularly in supporting vulnerable populations.
- Excellent communication and listening skills.
- Familiarity with local services and resources, including access methods.
- A genuine passion for helping others, demonstrating care and empathy.
- Ability to provide person-centered support in a respectful manner.
- Experience in conducting holistic care planning assessments.
- Background in primary care administration or management.
- Proficiency in using clinical systems such as SystmOne and EMIS.
- Experience working collaboratively with patients and multidisciplinary teams.
