About the Role

North Uttlesford PCN is pleased to announce an opportunity for a Community Health Link Facilitator to become part of our neighborhood team, available for full-time or part-time engagement.

This innovative position will function within the Cambridge Hospitals Foundation Trust as a member of the complex discharge team, aiming to streamline and enhance the discharge process for patients. This role is designed to collaborate with individual patients and their families to ensure effective discharge planning. It serves as a vital resource in voluntary care, leveraging community services to support post-acute hospital care.

Main Responsibilities
  • Proactively identify and engage with patients at Addenbrooke's Hospital who require additional assistance to ensure a successful and lasting discharge.
  • Deliver personalized support to vulnerable individuals.
  • Collaborate with GP practice teams and PCN colleagues to support patients and encourage positive lifestyle changes, aiming to prevent health issues through behavioral change techniques.
  • Conduct comprehensive care planning assessments, evaluating how needs can be met by local services and community opportunities.
  • Provide information and advice on a wide array of services to promote health and well-being, fostering independence within communities.
  • Establish strong connections with local voluntary, community, and social enterprise organizations.
  • Contribute to reducing the demand on statutory services, minimizing hospital readmissions, and addressing unnecessary GP appointments.
  • Work with patients in their homes to offer personalized, person-centered support following hospital discharge.

About the Candidate

If you are a dedicated community advocate or an experienced social prescriber seeking a new and rewarding challenge, we would be delighted to hear from you. The ideal candidate will possess the following qualifications:

  • Experience in healthcare, voluntary, or community sectors, supporting vulnerable populations.
  • Diploma Level 2 in Health and Social Care or an equivalent qualification or experience.
  • Excellent listening and communication skills.
  • Familiarity with local resources and services and the ability to access them.
  • A strong motivation to assist others, demonstrating care and empathy.
  • Capability to provide person-centered support in a non-judgmental manner.
  • Experience in conducting holistic, person-centered care planning assessments.
  • Understanding of the needs of small, volunteer-led community groups and the ability to foster their development.
  • Commitment to collaborative efforts with all local agencies.
  • Experience and confidence in delivering lifestyle change interventions.
About Us

Our primary care network (PCN) comprises four North Uttlesford practices working collaboratively. PCNs enhance existing primary care services, enabling the provision of proactive, personalized, coordinated, and integrated health and social care for individuals in their communities. Clinicians describe this approach as a shift from reactive appointment-based care to proactive support for the individuals and communities they serve.

We are a growing, supportive team led by our clinical director, along with GP and nurse educators, pharmacists, paramedics, care coordinators, health and wellbeing coaches, an integrated mental health team, and social prescribers.
