Job Requirements

Qualifications

Essential

  • A Diploma Level 2 in Health and Social Care or an equivalent qualification is preferred.

Experience

  • Proven experience in healthcare or community services, particularly in supporting vulnerable populations.
  • Strong communication and listening abilities.
  • Familiarity with local resources and the means to access them.
  • A genuine motivation to assist others, demonstrating care and empathy.
  • Ability to provide person-centered support in a respectful manner.
  • Experience in conducting holistic care planning assessments.
  • Background in primary care, particularly in a senior administrative or management capacity.
  • Proficiency in using clinical systems such as SystmOne and EMIS.
  • Experience working collaboratively with patients and as part of a multidisciplinary team.
