About the Role

We are seeking a dedicated and compassionate Care Home Assistant Practitioner to join our team at Four Seasons Health Care. As a key member of our care team, you will play a vital role in delivering high-quality, person-centred care to our residents.

Key Responsibilities
  • Assist the Registered Nurse in monitoring the standard of care and ensuring that all residents receive quality medical and personal care.
  • Monitor the wellbeing of each resident, identifying any changes that impact on care and care plans.
  • Support the Home Manager in implementing policies, procedures, and business objectives, including preparing staff duty rota's.
  • Identify training needs of the team and support the implementation of the home's training programme.
  • Conduct staff supervisions and appraisals, ensuring they meet external regulator guidelines.
  • Act as a focal point for contact with residents, their relatives, and visitors, as well as health and social care professionals.
Requirements
  • Possess a passion for providing quality care in our homes.
  • Have experience in a relevant care role and be able to evidence full competence.
  • Be qualified in, or working towards, a level three health and social care qualification.
  • Be committed to ongoing professional development.
  • Be up to date with all personal essential learning.
