About the Role

We are seeking a dedicated and compassionate Care Home Assistant Practitioner to join our team at Four Seasons Health Care. As a key member of our care team, you will play a vital role in delivering high-quality care to our residents.

Key Responsibilities
  • Assist the Registered Nurse in ensuring the delivery of person-centered care to all residents
  • Monitor the wellbeing of each resident and identify any changes that may impact on their care and care plans
  • Support the Home Manager in implementing policies, procedures, and business objectives
  • Conduct staff supervisions and appraisals to ensure they meet external regulator guidelines
  • Act as a focal point for contact with residents, their relatives, and visitors, as well as health and social care professionals
Requirements
  • Passionate about providing quality care in a care home setting
  • Experienced in a relevant care role and able to evidence full competence
  • Qualified in, or working towards, a level three health and social care qualification
  • Committed to ongoing professional development
  • Up to date with all personal essential learning
