Job Summary:

We are seeking a highly skilled and experienced Care Manager to join our team at Nursing Home, Pontypool, UK. As a key member of our management team, you will be responsible for overseeing the daily operations of the nursing home, ensuring high standards of nursing care and promoting residents' health, independence, and well-being.

Main Responsibilities:

  • Leadership and Management: Manage the admission and discharge of residents, ensuring compliance with nursing home policies and procedures.
  • Quality Assurance: Conduct audits of services and provisions, including care plan audits, medication audits, and falls risk assessments. Present results at regular management meetings.
  • Staff Management: Manage staff resources, including recruitment, supervision, appraisals, disciplinary actions, and rota development.
  • Resident Care: Assess and identify residents' needs and risks, develop and implement personalized care plans, and evaluate changing needs.
  • Communication: Build and maintain trust with residents, relatives, and other stakeholders, and communicate effectively with healthcare professionals.
  • Medication Management: Follow medication management and administration policies, including ordering, storage, administration, destruction, and reviewing medications.
  • Collaboration: Collaborate with professionals such as community psychiatric nurses, social workers, nurse assessors, consultants, and general practitioners.
  • Safeguarding: Safeguard vulnerable adults per care home policies and the All Wales Interim Policies and Procedures for the Protection of Vulnerable Adults from Abuse.
  • Leadership and Teamwork: Lead the team in organizing staff and prioritizing workloads, and foster good working relationships and teamwork among all staff levels.
  • Administrative Duties: Assist in maintaining the home's registration with CIW, help maintain provider status with local authorities, and support the maintenance of occupancy levels.
  • Professional Development: Stay updated on current policies and procedures, discuss and promote innovations to improve care or efficiency, and participate in professional societies.

Requirements:

  • Qualifications: Level 5 Leadership in Management and Adult Care or Registered Nurse Part 1 (RN1, RNA, RMNH, RNLD)
  • Driving Licence: Required
