Welcome to FCMS

We are seeking an individual to deliver administrative assistance to the Patient Care Coordination Service. This role is primarily office-based but may require occasional visits to various operational sites. The Patient Care Coordination Service operates around the clock, necessitating flexibility in working hours to meet service demands.

COMPENSATION: Competitive hourly rate with additional pay for weekend and overnight shifts.

Key Responsibilities

  • Provide administrative support to the Patient Care Coordination Service.
  • Deliver a prompt, precise, and efficient telephonic service for patients.
  • Assist clinical personnel with administrative tasks.
  • Participate in a comprehensive internal training program.
  • Utilize relevant software effectively and safely, engaging in ongoing training for professional development.
  • Accurately record and process information received, ensuring data is logged appropriately using recognized software.
  • Employ empathetic communication over the phone to gather essential information.
  • Input sensitive patient data, including end-of-life care and chronic health conditions, with precision.
  • Maintain composure under pressure while being sensitive to the needs of callers and patients.
  • Complete all necessary training in line with job requirements.
  • Handle personal and confidential information with care, adhering to Information Governance Policies.
  • Perform clerical duties, data entry, and other tasks relevant to FCMS operations, following established protocols.
  • Collaborate effectively with external organizations, including healthcare providers and community services.
  • Provide clear and concise information to patients and external partners.
  • Support the broader team in fulfilling the responsibilities of the System Deflections team as needed.
  • Conduct oneself in a courteous, empathetic, and professional manner at all times.
  • Attend meetings as required.
  • Assist management as necessary.
  • Exhibit flexibility in working various shifts, including daytime, evenings, weekends, and holidays, to cover for absences.
  • Adhere to FCMS Policies and Procedures consistently.

Physical and Mental Demands

Staff typically remain seated at their desks for the majority of the workday. The role may involve interruptions from phone calls or inquiries from colleagues, requiring the ability to prioritize tasks effectively to meet deadlines. The nature of the work can vary significantly, ranging from assisting individuals with complex health needs to routine administrative tasks.

Emotional Considerations

The position requires a high degree of tact and diplomacy when addressing inquiries from patients, families, and healthcare professionals. Staff must be prepared to support vulnerable individuals and manage emotionally charged situations with composure.

Information Management

Collaborate with IT support to ensure effective management of information systems within the service.

Compliance and Governance

Understand the significance of Information Governance and maintain compliance with the IG Toolkit annually. Ensure accurate statistical reporting in line with legal and organizational requirements.

Training and Development

Identify personal training needs and engage in relevant educational opportunities. Ensure all mandatory training is completed and maintained.

Additional Responsibilities
