Position Overview

Stonyhill Medical Practice is seeking a dedicated and knowledgeable Pharmaceutical Care Specialist to become a vital member of our collaborative healthcare team.

This position offers the potential for full-time engagement.

The successful candidate will have the opportunity to work closely with a diverse primary care team, providing outstanding healthcare services to a patient population of 12,500.

This role will leverage a broad range of competencies in delivering direct patient care and will be instrumental in the comprehensive management of medications within the practice.

Compensation and working hours will be tailored to the qualifications of the selected candidate.

Key Responsibilities

As a member of the pharmacy team, you will collaborate with the lead Pharmacist, PCN Pharmacist, Pharmacy Technician, Prescription Clerks, and Administration Assistant.

Core Duties Include:

  • Assisting the GP practice in enhancing the clinical pharmacist role under the mentorship of the Lead Pharmacist.
  • Promoting and implementing high-quality, evidence-based, and safe medication practices.
  • Engaging in patient-facing clinical activities, including medication assessments, optimization, and reconciliation, while working towards achieving prescribing objectives.
  • Serving as a specialized resource for the GP practice, addressing medication-related inquiries, and coordinating with other healthcare professionals as necessary.
  • Supporting the development and execution of various projects within the practice.
  • Conducting clinical medication evaluations for patients with complex health conditions and making independent prescribing decisions.
  • Managing a personal caseload of patients with medication-related challenges, including those at high risk and those in care facilities.
  • Performing clinical reviews for patients susceptible to hospital admissions due to medication issues.
  • Collaborating with primary healthcare colleagues to enhance patient medication regimens, including the cessation of unnecessary medications.
  • Assisting patients with chronic conditions and prescribing within the scope of expertise.
  • Addressing and documenting complex medication inquiries from both healthcare professionals and the public, utilizing professional judgment.
  • Managing patient queries or complaints regarding medication changes, ensuring adherence to governance protocols.
  • Collaborating with colleagues to develop, implement, and evaluate repeat prescribing policies and procedures.
  • Reconciling medications post-discharge from healthcare facilities, ensuring continuity of care.
  • Working with hospital teams to proactively manage patients at risk of medication-related complications.
  • Identifying patient groups at heightened risk of medication-related harm.
  • Collaborating with patients and the primary care team to mitigate risks through effective medication management.
  • Conducting practice searches to identify patients at risk of unplanned admissions due to medication issues.
  • Engaging with multidisciplinary teams to manage medication-related risks effectively.
  • Implementing strategies to reduce the prescribing of high-risk medications.
  • Contributing to the growth and development of the pharmacy team through shared learning and regular updates.
  • Participating in routine role evaluations with the Lead Pharmacist.
  • Identifying areas for improvement in prescribing practices.
  • Conducting audits and quality improvement initiatives.
  • Guiding audit activities within the pharmacy team.
  • Collaborating to establish a work plan focusing on key development areas.
  • Providing guidance to ensure compliance with relevant standards in medication management.
  • Understanding and acting on reports related to the practice's prescribing budget and project outcomes.

Qualifications and Skills

Essential Qualifications:

  • Independent Prescribing qualification.
  • Postgraduate diploma/MSc in Clinical Pharmacy or equivalent.
  • Master's degree (or equivalent) in Pharmacy.
  • Membership of the General Pharmaceutical Council for Pharmacists.
  • Evidence of Continuing Professional Development.
