About the Role

We are seeking a highly motivated and experienced Senior End of Life Care Specialist to join our team at Blackpool Teaching Hospitals NHS Foundation Trust. As a key member of our End of Life and Bereavement Care Team, you will play a pivotal role in developing and delivering high-quality end of life care services to patients and their families.

Key Responsibilities
  • Develop and implement the Swan Model of Care across the organization, working closely with end of life, palliative care, chaplaincy, and bereavement services.
  • Provide bereavement support and education to patients, families, and healthcare staff, ensuring a patient-centered approach to care.
  • Collaborate with clinical teams to develop and deliver education and training programs on end of life care, ensuring all staff have the necessary knowledge and skills to provide high-quality care.
  • Lead and manage a team of clinical staff, providing guidance and support to ensure the delivery of excellent patient care.
  • Develop and maintain relationships with community partners, ensuring seamless care transitions and effective communication.
Requirements
  • Registered Nurse or AHP with relevant post-registration experience in end of life care.
  • Advanced communication skills, with the ability to lead and manage clinical staff.
  • Knowledge of the Swan Model of Care, national bereavement and end of life agenda, and CQC regulatory requirements.
  • Ability to lead and coordinate research and audit activity, with a focus on improving end of life care services.
  • Excellent verbal and written communication skills, with the ability to review performance and development of staff.
