About the Role

We are seeking a skilled and compassionate Healthcare Assistant to join our team at Trinity Hospice. As a key member of our Hospice at Home Team, you will provide high-quality care and support to patients and their families in their own homes.

Key Responsibilities
  • Work alongside Registered Nurses (RGNs) to deliver care and support to patients and their families in their homes.
  • Provide practical, emotional, and psychological support to patients and their families.
  • Support the management of symptoms associated with the end-of-life phase, including medication management.
  • Comply with all Trinity Hospice and Palliative Care Services policies and procedures.
  • Maintain a high standard of patient care, adhering to policies and procedures.
  • Develop and maintain specialist skills in palliative care relevant to the role.
  • Work a flexible shift pattern, covering a 24-hour rota as needed, and during quiet periods on the inpatient unit.
  • Work effectively and efficiently with community-based healthcare professionals (GPs, DNs/CMs).
  • Share responsibility with community-based healthcare professionals for planning, implementing, and evaluating patient care.
  • Monitor patient care plans and provide individualized patient care.
  • Communicate effectively and accurately with the Trinity Hospice at Home Team and community healthcare professionals.
  • Participate in multidisciplinary team working and handover of care from nights to days.
  • Support family/carers as patients' conditions deteriorate and death may be imminent.
  • Provide last offices after patient death.
  • Support and inform families and carers after patient death.
  • Communicate and liaise with Funeral Home staff to facilitate dignified transition of patient to Funeral Home.
  • Ensure correct use and economy of equipment and resources.
  • Ensure accurate and timely completion of documentation.
  • Represent Trinity Community Nursing Service in a professional and compassionate manner.

We are looking for a candidate with the following skills and attributes:

  • Able to communicate well verbally and in writing.
  • Ability to follow instructions.
  • Basic IT skills, including Word (able to log-in, use email, check calendars, etc., and log out).
  • Able to physically carry out duties of this role.
  • Able to follow instructions and use own initiative.
  • Willingness to learn new tasks, procedures, or methods.
  • Pleasant, warm personality with a kind and sympathetic attitude.
  • Self-motivated.
  • Demonstrate a commitment to caring for people.
  • Ability to adjust to change.
  • Flexibility regarding working rota.
  • UK driving license and access to car.
  • Commitment to the vision, values, and objectives of Trinity Hospice.
  • Ability to work unsociable hours.
  • English language GCSE Grade C or above or equivalent.
  • NVQ 2 or equivalent experience.
  • Three years' experience in a healthcare setting.
  • Ability to establish good working relationships and work well in a team.
