About the Role

The South West Yorkshire Partnership NHS Trust is seeking a highly skilled and experienced Senior Liaison and Diversion Practitioner to join our team. As a key member of our multi-disciplinary team, you will be responsible for providing intensive mental health and social care assessments, identifying care pathways, and diverting individuals within the police custody suites, courts, and probation services.

Key Responsibilities
  • Attend police custody suites and magistrates courts to provide mental health assessments, advice, and identify robust care pathways for individuals with potential mental health or learning disability needs.
  • Provide written information to the police and court in a timely fashion to inform disposal.
  • Work closely with referring agencies within the custody suite, court, and probation, acting as a single point of contact for health.
  • Address the court in person when necessary to advise on the disposal and care pathway of a particular case.
  • Facilitate mental health act assessments at the police station when diversion is indicated.
Requirements
  • Working knowledge of the Mental Health Act 1983, Care Programme Approach, and other relevant codes of practice.
  • Experience of working with people who are difficult to engage, having developed through theoretical and practical knowledge of working with people with severe and enduring mental illness.
  • Ability to be flexible and adapt to changing situations.
  • Substantial experience of practice at a senior level, working autonomously and responsible for supervising others.
  • Substantial experience in community settings.
  • Evidence of leadership skills and experience of supervising other staff.
  • Experience of supporting carers and families.
  • A proven commitment to working in partnership with service users and carers in the delivery of effective services.
Desirable Qualifications and Experience
  • Experience or training in research.
  • Recognised qualification in clinical supervision.
  • Experience of substance misuse.
  • Relapse prevention.
  • Use of assessment tools.
Working Arrangements

This is a full-time position, and the successful candidate will be required to work unsocial hours. A current driving license and access to a car during working hours are essential (reasonable adjustments will be considered for any applicants who are unable to drive due to a disability).
