Position Overview:

The role is situated within collaborative offices alongside health professionals, either at designated healthcare facilities or centralized access points. The focus is on facilitating the transition of individuals from hospital settings back to their homes, particularly those with health and social care requirements.

This position necessitates close collaboration with healthcare colleagues to ensure timely discharge from hospitals, both within and outside the borough, where the Local Authority has obligations to address care needs.

Primary Responsibilities:

1. Fulfill the Authority's obligations under applicable legislation concerning service users with disabilities and care needs.

2. Adhere to the council's eligibility criteria, policies, and procedures regarding service users.

3. Develop support packages aimed at preventing unnecessary admissions to care facilities and assisting individuals in maintaining their independence at home or transitioning to appropriate residential care.

4. Align service user requirements with suitable placements or service providers.

Required Qualifications and Experience:

1. Proficiency in computer applications and administrative functions is essential.

2. Familiarity with Care Standards, relevant legislation, and the CQC registration process is crucial.

3. Proven experience in negotiating contracts or agreements is required.

4. Background in a customer-oriented environment is necessary.

5. Demonstrated commitment to ongoing professional and personal development is expected.

Educational Requirements:

A minimum of A-level education or equivalent is essential.

Additional Considerations:

1. Weekly working hours: 22 hours.

2. A current standard CRB/DBS clearance is mandatory.

3. The role may require weekend shifts, with a typical schedule spanning across two weeks.
