Position Overview:

The role is situated within integrated offices alongside health professionals, focusing on facilitating the transition of individuals from hospital to home. This position is pivotal in ensuring that individuals with health and social care requirements receive appropriate support upon discharge.

Key Responsibilities:

· Collaborate with health colleagues to source and arrange domiciliary care packages for patients returning home from hospital.

· Work in conjunction with various stakeholders to facilitate discharges from hospitals, ensuring that the needs of the London Borough of Bromley are met effectively.

· Uphold the Authority's obligations under the Adult Social Care Outcomes Framework (ASCOF) and relevant legislation, particularly for service users with disabilities and care needs.

· Ensure compliance with the council's eligibility criteria, policies, and procedures regarding service users.

· Develop support packages aimed at preventing unnecessary admissions to care facilities and promoting independent living for individuals.

· Assess and match service user requirements with suitable placements or service providers.

Essential Qualifications and Experience:

· Proficiency in administrative tasks and familiarity with computer software is essential.

· Knowledge of Care Standards, relevant legislation, and the Care Quality Commission (CQC) registration process is crucial.

· Experience in negotiating contracts or agreements is required.

· A background in customer-focused environments is necessary.

· Evidence of ongoing professional development is essential.

Educational Requirements:

· A minimum of A-level education is required.

Additional Information:

· Weekly working hours: 22 hours.

· A current standard CRB/DBS clearance is mandatory for this role.

· Weekend availability is required, with a typical schedule covering various days across two weeks.
