Job Summary:

The Placement Officer will be responsible for sourcing care support packages to enable individuals to return home from hospital, where they have health and social care needs. This role involves working closely with colleagues from the hospital and other healthcare providers to facilitate discharge from hospital and ensure that the individual's needs are met.

Key Responsibilities:

  • To discharge the council's responsibilities under the Adult Social Care Outcomes Framework (ASCOF) and other relevant legislation, with respect to individuals with disabilities, vulnerabilities, and eligible care needs.
  • To ensure that the council's eligibility criteria, policies, and procedures are followed in relation to service users.
  • To create packages of support to prevent unnecessary admissions to care and to assist individuals to remain in their own home where possible, or to move to suitable and appropriate residential care.
  • To match service user needs to potential placements or providers.

Essential Requirements:

  • Experience of using computer packages and undertaking administrative tasks.
  • Experience of care standards and relevant legislation, including the CQC registration and approval process.
  • Experience of negotiating agreements or contracts.
  • Previous experience of a customer-focused environment.
  • Evidence of continued professional and personal development.

Essential Qualifications:

  • High-level education, such as A-levels or equivalent.

Working Arrangements:

  • 22 hours per week.
  • Current standard CRB/DBS clearance required.
  • Weekend working may be required, including Saturday and Sunday.
