Position Overview

The Hepatology Division operates within the Gastroenterology Department, which is a key component of the Gastrointestinal Medicine and Surgery (GMS) Directorate. This department addresses the comprehensive gastrointestinal requirements of the local community while also catering to specialized needs for broader regions.

Guy's and St Thomas' Trust is recognized as a Cancer Centre, focusing on upper gastrointestinal malignancies, head and neck cancers, and, in collaboration with King's College Hospital, pancreatic and biliary cancers. The department has established expertise in various areas, including inflammatory bowel disease, interventional pancreato-biliary endoscopy, endoscopic ultrasound, and liver disease management. The liver unit has experienced consistent growth and has recently attained national accreditation for its service quality. Currently, the unit is staffed by five permanent consultants and one locum consultant, facing a steady rise in both routine and urgent patient referrals.

Key Responsibilities

The primary objective of this role is to deliver exceptional care for the increasing number of patients with liver and biliary disorders requiring outpatient or inpatient evaluations. Additionally, support for the general operations of the Endoscopy Unit is a vital aspect of this position. The successful candidate will also provide senior clinical guidance to the hepatology multidisciplinary team and play a significant role in the education and training of specialist registrars, fellows, and other team members.

The hepatology role encompasses a substantial community component, which includes conducting clinics and participating in multidisciplinary team meetings within the community.

It is essential for the candidate to possess full colonoscopy certification prior to commencing this role.

This position includes a General Internal Medicine (GIM) component, which entails weekend on-call duties, twilight on-call responsibilities, and post-take ward rounds.

There is a commitment to cover eight weekend days, which includes providing endoscopy and GIM services.

The primary aim of this position is to ensure high-quality care for the growing number of patients with liver and biliary diseases referred for outpatient or inpatient assessments. Support for the Endoscopy Unit's general needs is also a critical part of this role. The post holder is expected to provide senior clinical support to the hepatology multidisciplinary team and will have a pivotal role in training specialist registrars and fellows, as well as other team members.

The hepatology role also includes a significant community aspect, involving clinics and multidisciplinary team meetings.

It is required that the post holder has full colonoscopy sign-off before starting in this position.

This role includes a GIM component, which involves weekend on-call duties, twilight on-call responsibilities, and post-take ward rounds.

There is an eight-weekend day commitment, which includes endoscopy and GIM responsibilities.

Qualifications and Experience

Essential

  • MBBS (or equivalent) with GMC registration and a license to practice
  • MRCP or equivalent
  • Completion of specialist training in gastroenterology (or within six months of CCT date)

Desirable

  • Higher degree (MD, PhD)

Clinical Experience

Essential

  • Comprehensive clinical experience in all aspects of Gastroenterology
  • Documented experience in hepatology (minimum of six months)

Desirable

  • Broad-ranging experience in a tertiary hepatology unit

Knowledge and Skills

Essential

  • Ability to function effectively as part of a multidisciplinary team
  • Fully competent in upper and lower GI endoscopy with documented performance indicators
  • Fully competent in the assessment of hepatitis B and C

Desirable

  • Documented experience and ability to perform advanced therapeutic endoscopy, endoscopic management of portal hypertension, or abdominal ultrasound.

Management Skills

Essential

  • Evidence of understanding the role of clinical management
  • Documented experience in leading quality improvement initiatives
  • Evidence of contribution to effective clinical audit and clinical risk management
  • Attendance at leadership and management training

Desirable

  • Development of clinical guidelines
  • Experience in management and quality improvement, particularly in hepatology

  • Evidence of understanding and adherence to the principles of Good Medical Practice as outlined by the General Medical Council
  • Evidence of contribution to effective clinical audit and clinical risk management
