Care Navigator

About the Role
We are seeking a skilled and compassionate Care Navigator to join our team at Tile House Surgery. As a Care Navigator, you will play a vital role in ensuring the smooth operation of our practice, providing exceptional patient care and support.
Main Responsibilities
- Provide a warm and welcoming experience for patients, responding to their queries and concerns in a polite and professional manner.
- Manage patient appointments, bookings, and cancellations, ensuring seamless communication with our clinical team.
- Handle patient correspondence, including letters, forms, and specimen bottles, maintaining confidentiality and discretion at all times.
- Assist patients with information and resolve problems and complaints in a timely and effective manner.
- Advise patients on practice procedures and policies, promoting a positive and supportive environment.
- Register new patients, ensuring accurate and up-to-date records.
- Answer telephone calls promptly and efficiently, providing clear and concise information to patients and staff.
- Support GPs with their requests, maintaining a high level of professionalism and attention to detail.
- Refer patients to local domiciliary services, ensuring seamless transitions and continuity of care.
- Manage incoming emails and faxes, adhering to practice procedures and protocols.
