About the Role

We are seeking a skilled and compassionate Care Navigator to join our team at Tile House Surgery. As a Care Navigator, you will play a vital role in ensuring the smooth operation of our practice, providing exceptional patient care and support.

Main Responsibilities

  • Provide a warm and welcoming experience for patients, responding to their queries and concerns in a polite and professional manner.
  • Manage patient appointments, bookings, and cancellations, ensuring seamless communication with our clinical team.
  • Handle patient correspondence, including letters, forms, and specimen bottles, maintaining confidentiality and discretion at all times.
  • Respond to patient requests for information, resolving problems and complaints in a timely and effective manner.
  • Advise patients on practice procedures, ensuring they are informed and empowered to make informed decisions about their care.
  • Register new patients, verifying their details and ensuring accurate records are maintained.
  • Answer telephone calls promptly, providing clear and concise information to patients and staff.
  • Support GPs with their requests, working collaboratively to deliver high-quality patient care.
  • Refer patients to local domiciliary services, ensuring they receive the support they need.
  • Manage incoming emails and faxes, adhering to practice procedures and protocols.

Person Specification

We are looking for an experienced and skilled individual with:

  • Previous medical or healthcare experience, customer service, and/or receptionist experience.
  • Excellent communication skills, with a clear and polite telephone manner.
  • Ability to work in a polite, confidential, and discreet manner, respecting patients at all times.
  • Experience working with the general public, being sensitive and empathetic in distressing situations.
  • Confidence to work as part of a team and think on their feet independently.
  • Strong IT and administration skills.
