Cera Care is seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in ensuring the delivery of high-quality care services to our clients.
Key Responsibilities:
- Manage the delivery of care services to clients, ensuring that care workers are matched to clients based on their needs and preferences.
- Conduct care assessments and reviews to ensure that clients receive the most appropriate care.
- Develop and implement care plans that meet the unique needs of each client.
- Collaborate with care workers to ensure that they have the necessary skills and knowledge to provide high-quality care.
- Monitor and evaluate the quality of care services, making recommendations for improvement as needed.
Requirements:
- Relevant qualifications, such as NVQ/SVQ 2 or 3, and registration with the SSSC.
- At least 2 years of experience working as a care coordinator for a domiciliary care agency.
- Knowledge of CQC/CI/CIW compliance and regulations.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
