About the Role
We are seeking a highly organized and communicative Care Navigator to join our team at Tile House Surgery. As a Care Navigator, you will play a vital role in ensuring the smooth operation of our practice, providing exceptional patient care and support.
Main Responsibilities

- Manage patient appointments, bookings, and cancellations, ensuring seamless communication with our clinical team.

- Develop strong relationships with patients, carers, and other healthcare professionals to ensure effective communication and coordination of care.
