About the Role
We are seeking a highly organized and communicative Care Navigator to join our team at Tile House Surgery. As a Care Navigator, you will play a vital role in ensuring the smooth operation of our practice, providing exceptional patient care and support.
Main Responsibilities
- Provide a warm and welcoming experience for patients, responding to their queries and concerns in a polite and professional manner.
- Manage patient appointments, including booking, cancelling, and moving appointments as needed.
- Handle administrative tasks, such as distributing letters, completed forms, and specimen bottles.
- Deal with requests for information, manage problems and complaints, and advise patients on practice procedures.
- Register new patients and maintain accurate records.
- Answer phone calls promptly and efficiently, providing clear and concise information to patients.
- Assist GPs with their requests and refer patients to local domiciliary services as needed.
- Manage incoming emails and faxes according to practice procedure.

Tile House Surgery is a large training practice serving a patient population of 13,000. We pride ourselves on providing high-quality care to our patients and staff, operating the SystmOne clinical system and achieving high QOF achievement and a CQC rating of 'Good' across all domains.
