About the Role
We are seeking a skilled Care Navigator to join our team at Tile House Surgery. As a Care Navigator, you will play a vital role in ensuring the smooth operation of our practice, providing exceptional patient care and support.
Main Responsibilities
- Provide a warm and welcoming experience for patients, responding to their queries and concerns in a polite and professional manner.
- Manage patient appointments, bookings, and cancellations, ensuring seamless communication with our clinical team.
- Handle patient correspondence, including letters, forms, and specimen bottles, maintaining confidentiality and discretion at all times.
- Respond to patient requests for information, resolving problems and complaints in a timely and effective manner.
- Advise patients on practice procedures, ensuring they are aware of their rights and responsibilities.
- Register new patients, verifying their details and ensuring accurate records are maintained.
- Answer telephone calls promptly, providing clear and concise information to patients and staff.
- Assist GPs with their requests, supporting the delivery of high-quality patient care.
- Refer patients to local domiciliary services, ensuring they receive the support they need.
- Manage incoming emails and faxes, adhering to practice procedures and protocols.

Person Specification
We are looking for an experienced Care Navigator with:
Previous medical or healthcare experience, customer service, and/or receptionist experience.Excellent communication skills, with a clear and polite telephone manner.Ability to work in a polite, confidential, and discreet manner, respecting patients at all times.Experience of working with the general public, being sensitive and empathic in distressing situations.Confidence to work as part of a team and think on your feet independently.Strong IT and administration skills.
