About the Role

We are seeking a highly skilled and motivated individual to join our Learning Disability team as a Qualified Practitioner Level One or Two. As a key member of our team, you will be responsible for working with service users and their families to maximise independence and support people to reach their goals and aspirations.

Key responsibilities will include carrying out strength-based Adult Needs Assessments and Reviews, working in a Person-Centred way, and developing and implementing support plans to meet the needs of service users. You will also be required to travel to appointments to meet the needs of the service, which may include out-of-county travel.

We are looking for someone who is passionate about providing high-quality support to people with learning disabilities and their families. You will need to have excellent communication skills, be able to work effectively in high-pressure situations, and have a positive approach with self-motivation and enthusiasm.

About You

We are looking for someone who has a Social Work or Nursing qualification and is registered with the relevant professional body. You will need to have a strong understanding of the principles of Person-Centred practice and be able to demonstrate your ability to work in a strengths-based way.

You will also need to have excellent communication and interpersonal skills, be able to work effectively in a team, and have a positive approach with self-motivation and enthusiasm.
