We are seeking a highly skilled and motivated individual to join our Learning Disability team as a Qualified Practitioner Level One or Two. As a key member of our team, you will be responsible for working with service users and their families to maximise independence and support people to reach their goals and aspirations.
Key responsibilities will include carrying out strength-based Adult Needs Assessments and Reviews, working in a Person-Centred way, and developing effective plans to support service users. You will also be required to travel to appointments to meet the needs of the service, which may involve out-of-county travel.
We are looking for someone who is passionate about providing high-quality support and has excellent communication skills. You will need to be able to work effectively in high-pressure situations and have a positive approach with self-motivation and enthusiasm.
