About the Role

We are seeking a highly skilled and motivated Practitioner to join our Adult Social Care team. As a Practitioner, you will be responsible for working with individuals with complex needs, providing person-centred support and guidance to help them achieve their goals and aspirations.

Key responsibilities include:

  • Conducting Adult Needs Assessments and Reviews to identify individual needs and develop personalized support plans
  • Working collaboratively with service users, families, and other professionals to maximize independence and promote community access
  • Providing emotional support and guidance to individuals with complex needs, including those with learning disabilities
  • Traveling to appointments and meetings to meet the needs of the service, including out-of-county travel
  • Working effectively in a high-pressure environment, responding to last-minute unplanned visits and meetings
