We are pleased to announce an opportunity for a Healthcare Practitioner in Primary Care to become a vital member of our healthcare team. This role is situated within a secure environment, where you will collaborate with a diverse group of professionals dedicated to providing exceptional care.

In this position, you will engage with a multi-disciplinary team that is committed to your professional growth and success.

The facility operates a comprehensive service with various shift patterns:

  • Weekdays: Standard shifts
  • Weekends: 8 AM - onwards
  • Evening shifts: 6 PM - onwards
  • Long day shifts: 9 AM - 9 PM
  • Night shifts: Overnight

Shifts are organized on a rolling four-week schedule, requiring weekend and holiday availability.

This is a full-time role with a competitive annual salary of up to £41,000, commensurate with experience.

Key Responsibilities

As a Healthcare Practitioner in Primary Care, your duties will encompass:

  • Delivering a nurse-led service aimed at minimizing hospital admissions
  • Conducting medication rounds, managing controlled substances, responding to emergencies, assessing patients, and performing initial screenings
  • Providing compassionate, non-judgmental care
  • Establishing strong partnerships with stakeholders

Qualifications Required

  • Registered Nurse (Adult)
  • Current professional registration (NMC)
  • Understanding of multi-disciplinary and multi-agency collaboration
  • A compassionate and non-judgmental approach
  • Knowledge of clinical governance, health and safety standards, and national guidelines

While not mandatory, experience in a hospital, community, or secure environment is preferred.
