Position Overview

An exciting opportunity has emerged for Band 4 Community Therapy Support Practitioners to become integral members of our evolving Community Admission Avoidance Therapy Team (CAATT).

We have multiple Community Therapy Support Practitioner roles available across various healthcare facilities. The contractual hours will be discussed during the interview process and can be negotiated upon successful appointment, with both full-time and part-time options available. While successful candidates will have a primary location, there will be an expectation to work across different sites as needed, with prior notice provided. This flexibility is essential to ensure that our services remain responsive to patient needs and that we support 7-day working schedules.

The role involves contributing to the CAATT by delivering high-quality, effective assistance to individuals who have accessed emergency care services. This work aligns with the ABUHB Clinical Futures initiative aimed at preventing unnecessary hospital admissions, minimizing hospital stay durations, and providing care closer to home by collaborating with primary, community, and third-sector organizations.

Main Responsibilities

This position is recognized as a vital component of the multidisciplinary service delivery, and we seek skilled, organized, and enthusiastic individuals to join our team. Responsibilities will include:

  • Delivering therapeutic interventions as delegated by registered therapists.
  • Maintaining close communication with patients, their families, members of the multidisciplinary team (MDT), and external organizations.
  • Demonstrating effective time management, prioritization skills, and adaptability in approach.
  • Communicating effectively, both verbally and in writing.

Successful candidates will collaborate with registered Occupational Therapists, Physiotherapists, and Dietitians, managing a delegated caseload of patients. The role will encompass supporting the assessment and intervention plans for individuals with various functional challenges, including those with multiple co-morbidities, frailty, and cognitive impairments.

Some positions may require providing a flexible 7-day, extended hour service as the program develops.

Welsh language proficiency is desirable for this role; applications are welcome from both Welsh and English speakers.

Qualifications and Experience
