Brackenberry Limited is collaborating with a Local Authority to recruit a Community Support Officer for an 8-month contract, with the possibility of extension based on client needs.

Compensation:
£20.02 per hour


Key Responsibilities:
  • Operate autonomously under the guidance of the Team Leader, ensuring deadlines are met while managing shifting priorities and circumstances.
  • Deliver essential services to protect vulnerable adults and their families, tailored to their assessed requirements.
  • Conduct comprehensive assessments regarding the needs of adults and their caregivers, adhering to the department's eligibility criteria and national performance standards within designated timeframes.
  • Develop and execute culturally sensitive care plans and services that reflect adaptability and innovative resource utilization, while fulfilling a care coordination role.
  • Manage case responsibilities, including proactive caseload management (with a focus on risk management), monitoring, coordinating, and reviewing care plans and ongoing service delivery.

  • Immediate availability or short notice is required.
  • Eligibility to work in the U.K. is essential.
  • This position necessitates an enhanced DBS check.
