Position Overview: We are seeking a dedicated Community Care Officer to be part of a vibrant Discharge to Assess Team, serving the Nottinghamshire South region.
Team Dynamics: Our team comprises Occupational Therapists, Community Care Officers, and Social Workers, all focused on ensuring safe hospital discharges with appropriate support. We aim to identify ongoing rehabilitation needs, empowering individuals to maintain their independence.
Service Structure: The Acute/sub-acute services include the Intermediate Discharge Team (IDT) and the Discharge to Assess Team (D2A). This role is within the D2A team, with a requirement for flexible working during peak times.
Key Responsibilities:
- Conduct assessments through engaging conversations.
- Commission services and oversee care packages within designated timelines.
- Possess a thorough understanding of community care legislation, including The Care Act 2014 and the Mental Capacity Act 2005.
- Commit to delivering high-quality services with ongoing training as a core component of the role.
Service Improvement: You will play a vital role in enhancing service delivery by incorporating Strengths Based Approaches, evidence-based assessments, and outcome-focused personal budgets.
