Job Title: Recovery Service Coordinator - Mental Health Practitioner

Job Summary:

Creative Support is seeking a highly skilled and experienced Recovery Service Coordinator - Mental Health Practitioner to join our team. As a key member of our supported living service, you will be responsible for leading the delivery of high-quality mental health services to our service users.

Key Responsibilities:

  • Lead the development and implementation of person-centered recovery plans, working closely with service users, their families, and other agencies to assess needs and plan successful transitions into the service.
  • Ensure that individuals are enabled to make progress in their personal recovery journey, attaining goals related to mental health and wellbeing, independence, tenancy maintenance, quality of life, and community connections.
  • Co-produce outcome-based support plans that build on strengths and assets, promoting meaningful occupation, resilience, and capacity for self-management.
  • Promote a welcoming and inclusive atmosphere, ensuring that staff are skilled in promoting recovery and positive engagement, with relationships based on respect and unconditional positive regard.
  • Ensure a strong commitment to equality and diversity, community asset-based approaches, and collaboration with community partners, including statutory mental health services and voluntary agencies.
  • Represent the organization and promote the service to referrers and potential service users.
  • Liaise with and develop positive relationships with housing providers, responsible for regular reporting and compliance for all aspects of property management.
  • Assess, support plan, and manage risk, conducting monthly key working sessions tailored to individual needs.
  • Mentor, supervise, and support staff, including staff development and training.
  • Organize person-centered reviews, enabling pathways to independence and move-on as appropriate.
  • Build positive partnerships with all stakeholders.

Requirements:

  • Relevant experience of working with people with mental health needs within a person-centered, recovery-focused framework.
  • Demonstrated knowledge of mental health needs/conditions, recovery principles, and helpful interventions.
  • Ability to work collaboratively with service users, their families, housing, and other agencies.
  • Strong leadership and management skills, with experience of line management and staff deployment.
  • Excellent communication and interpersonal skills, with the ability to promote a welcoming and inclusive atmosphere.
  • Commitment to equality and diversity, community asset-based approaches, and collaboration with community partners.
