About the Role

We are seeking a highly skilled and motivated Registered Nursing Associate to join our LDA Hospital at Home team. As a key member of our multidisciplinary team, you will play a vital role in delivering intensive community support to young people and their families.

Key Responsibilities
  • Provide holistic and person-centred care and support for young people up to 18 years old in a variety of settings.
  • Work collaboratively with registered nursing staff, other health professionals, and families to develop and implement individualized care plans.
  • Assist in the management of the care pathway, ensuring that children and young people receive the most appropriate care in an inpatient setting or via an 'out of hospital' service.
  • Contribute to the development and implementation of new care models and services that meet the needs of our patients and families.
Requirements
  • Registered Nursing Associate on the NMC register.
  • Completion of Nursing Associate foundation degree qualification.
  • Previous experience of working with young people or adults with a learning disability or Autism.
  • Strong communication and interpersonal skills, with the ability to work effectively with families and other healthcare professionals.
