About the Role

We are seeking a highly skilled and motivated Registered Nursing Associate to join our LDA Hospital at Home team. As a key member of our multidisciplinary team, you will play a vital role in delivering intensive community support to young people and their families.

Key Responsibilities
  • Work on a shared caseload with the team and be supervised by a registered nurse
  • Record own work on electronic patient records
  • Contribute to nursing assessments and care planning
  • Monitor and feed back on the physical and mental well-being of service users
  • Signpost to and work in collaboration with appropriate health services
  • Deliver therapeutic interventions to meet the physical, emotional, and psychological needs of individuals and carers
  • Arrange patient and carer monitoring visits and attend as part of a planned package of care
  • Discuss and explain a plan of care with patients and families or carers
  • Write and review care plans under supervision from a registered nurse
Requirements
  • Registered Nursing Associate on the NMC register
  • Completion of Nursing Associate foundation degree qualification
  • Ability to recognise own limitations in the context of the nursing associate role
  • Understanding of the importance of following procedures and treatment plans
  • Ability to support, supervise, assess, and act as a role model to nursing associate students, other learners, and healthcare support workers
  • Previous experience of working with young people or adults with a learning disability or autism
