We are seeking a highly skilled and motivated Registered Nursing Associate to join our LDA Hospital at Home team. As a key member of our multidisciplinary team, you will play a vital role in delivering intensive community support to young people and their families.
Key Responsibilities- Provide holistic and person-centred care and support for young people up to 18 years old in a variety of settings.
- Work collaboratively with registered nursing staff, other health professionals, and families to deliver a wide range of nursing interventions.
- Assist in the management of the care pathway to ensure that children and young people are cared for in an inpatient setting or via an 'out of hospital' service as close to home as possible.
- Registered Nursing Associate on the NMC register.
- Completion of Nursing Associate foundation degree qualification.
- Previous experience of working with young people or adults with a learning disability or Autism.
