Position Overview

The Directorate is excited to present a remarkable opportunity for a dedicated Advanced Clinical Specialist to join the One Coventry Integrated Team, focusing on enhancing prehospital and consultant-led care pathways.

The ideal candidate will serve as an Advanced Clinical Specialist, aiming to minimize unnecessary hospital admissions while facilitating effective admission avoidance strategies. This role demands the application of advanced health assessment capabilities and a commitment to pursuing non-medical prescribing qualifications, enabling support for patients with diverse medical conditions, ultimately improving patient outcomes and managing complex healthcare needs within community environments.

You will be expected to exhibit critical thinking in clinical decision-making, identify service gaps, and contribute to service enhancement in alignment with both national and local healthcare strategies. Your responsibilities will include fostering teamwork and providing mentorship, education, and support across the multidisciplinary team.

Key Responsibilities

The successful candidate will be responsible for:

  1. Conducting thorough history-taking, clinical evaluations, and implementing a variety of care interventions for patients, including those with undiagnosed conditions.
  2. Utilizing specialized knowledge and clinical reasoning to plan and deliver interventions in collaboration with multidisciplinary team members across various settings (patient homes, workplaces, or other community locations).
  3. Directing, referring, and facilitating access to health, social, and third-sector services to decrease inappropriate A&E visits and enhance patient quality of life.
  4. Collaborating with others to support staff through change processes, employing influencing skills to drive improvements. Contributing to plans that reflect changes in practice and service delivery.
  5. Effectively managing caseloads, determining when patients are ready for discharge or transfer to other service providers.
  6. Triage referrals into the service and ensure that care interventions are executed to meet patient needs.
  7. Assessing risks associated with community care and implementing necessary mitigations to address identified risks.
  8. Participating in 7-day working schedules to ensure the delivery of essential services.

Essential Requirements

A valid driver's license is mandatory for this role.
