Position Overview

We are seeking a dedicated and proactive Band 5 Clinical Nurse Specialist to become a vital member of our Post-Transplant unit.

The Post-Transplant team operates from Monday to Friday, 08:30 AM to 07:00 PM, providing outpatient clinical evaluations and guidance to individuals who have undergone renal transplantation.

Opportunities for rotation within various sectors of Renal Services and the chance to pursue advanced specialized training are available.

Important Note

This position is not suitable for candidates who are required to complete the OSCE training program. Only applicants with current and valid UK NMC registration will be considered for this role.

Key Responsibilities

  • Responsible for personal accountability in line with the Code of Professional Conduct, contributing to corporate goals while adhering to local, Trust, and statutory guidelines and policies.
  • Effectively communicate clinical decisions, policies, and care pathways to patients, caregivers, and the multidisciplinary team, especially in situations where comprehension challenges arise.
  • Serve as a competent practitioner, providing education, assessment, and mentorship to students or junior staff as appropriate.

Job Duties

  • Conduct clinical assessments for a designated group of patients and manage a team, ensuring that care is planned, executed, and evaluated with patient involvement.
  • Organize interdepartmental activities to support patient care pathways effectively.
  • Demonstrate technical and practical skills to ensure optimal patient care management, including patient monitoring, equipment handling, and administration of agreed invasive procedures.
  • Engage in the renal rotational program to gain comprehensive experience in all areas of renal medicine and renal replacement therapies.
  • Utilize and ensure others utilize information and IT systems to maintain accurate and timely patient, workforce, and resource data.
  • Manage expected and unexpected clinical events that require significant physical effort based on patient needs, consulting with senior staff as necessary.
  • Provide support to patients, caregivers, and others during challenging situations in the clinical setting, including delivering difficult news or addressing unexpected events.
  • Maintain a clean and safe environment, ensuring compliance with Trust cleanliness, hygiene, and infection control standards at all times.
  • Promote adherence to Health and Safety policies designed to protect healthcare staff and service users from known risks.
