In this role, you will collaborate with the team to enhance team management and foster innovative practices. You will be accountable for assessing care requirements and developing, implementing, and evaluating comprehensive care strategies.

As a Twilight Community Nursing Specialist, you will be part of a dedicated team responsible for assessing, developing, implementing, and evaluating extensive care plans for patients, ensuring their privacy and dignity are respected. Key responsibilities include working alongside the District Nurse to spearhead team initiatives, advocating for patient health and well-being, and providing education and support to patients' families and caregivers. You will ensure effective communication, maintain clear documentation, and uphold high standards of evidence-based nursing care. Additionally, you will mentor team members, engage in clinical supervision, and maintain professional integrity in accordance with NMC guidelines, while also supporting the Clinical Governance and Risk Management strategies of Epsom and St Helier University Hospitals NHS Trust. Flexibility to work across various localities as required is essential.

Main Responsibilities

  • Collaborate as part of the team, undertaking lead responsibilities as agreed with the District Nurse.
  • Respect the individuality of all patients, ensuring their privacy and dignity at all times.
  • Evaluate individual care needs and implement holistic care programs.
  • Involve, support, inform, and educate families and caregivers.
  • Promote the health and well-being of patients at every opportunity.
  • Establish and maintain clear and comprehensive records in accordance with trust procedures.
  • Facilitate training and professional development for team members and students.
  • Act as a mentor and assessor for both qualified and unqualified staff.
  • Maintain and enhance standards of evidence-based nursing care.
  • Uphold personal professional and clinical integrity in line with NMC guidelines.
  • Support and mentor team members while participating in clinical supervision activities.
  • Work independently while maintaining close communication with senior team members, stepping in as required.
  • Ensure timely patient care and maintain accurate data entry and record-keeping.
  • Collaborate with all professionals and agencies involved in patient care.
