About the Role

We are seeking a highly motivated and enthusiastic Registered Nurse to join our innovative community nursing team at Epsom and St Helier University Hospitals NHS Trust. As a Community Health Nurse, you will play a vital role in providing high-quality care to patients in their own communities.

Key Responsibilities
  • Assess and deliver holistic care packages to patients in their own homes, care homes, and through in-reach in to hospitals.
  • Work closely with the community hospitals and specialist teams to ensure a whole system approach to working across Surrey Downs.
  • Provide ongoing care and support to patients, deploying the most appropriate staff member(s) in a coordinated and timely way to best use community resources.
  • Reduce risk of admission for patients by supporting them in their homes and through in-reach in to hospitals.
  • Work as part of a community focused trimuvirate (group of three) of Lead GP / Community; Lead Clinician and Operational Manager.
Requirements
  • Registered Adult Nurse with NMC
  • Valid Driving Licence (to drive within UK)
  • Access to a vehicle for work purposes
  • Ability to assess, deliver and evaluate quality care
  • Effective communicator
