Job Overview

We are seeking a highly motivated and enthusiastic Registered Nurse to join our innovative community nursing team at Leatherhead Primary Care Network. As a Community Nurse, you will play a vital role in providing high-quality care to patients in their own homes, promoting independence, and improving health outcomes.

Main Responsibilities
  • Assess and deliver holistic care packages to patients in their own homes
  • Work closely with the community hospitals and specialist teams to ensure a whole system approach to care
  • Provide ongoing care and support to patients, reducing the risk of admission to hospital
  • Deploy the most appropriate staff member(s) in a coordinated and timely way to best use community resources
  • Support people in their homes, care homes, and through in-reach into hospitals where necessary

Person Specification

We are looking for a Registered Adult Nurse with NMC registration and a valid driving licence. Relevant post-registration experience and community experience are desirable. You must have access to a vehicle for work purposes and be able to assess, deliver, and evaluate quality care. Effective communication skills are essential.
