About the Role

We are seeking a highly motivated and enthusiastic Registered Nurse to join our innovative community nursing team at Epsom and St Helier University Hospitals NHS Trust. As a Community Health Nurse, you will play a vital role in providing high-quality care to patients in their own homes and communities.

Key Responsibilities
  • Assess and deliver holistic care packages to patients in their own homes and communities.
  • Work closely with the Primary Care Network (PCN) team to provide ongoing care and support to patients.
  • Deploy the most appropriate staff member(s) in a coordinated and timely way to best use community resources.
  • Reduce risk of admission for patients by providing proactive care and support.
  • Support people in their homes, care homes, and through in-reach into hospitals where necessary.
  • Work closely with specialist services and community hospitals to ensure a whole system approach to working across the trust.
Requirements
  • Registered Adult Nurse with NMC registration.
  • Valid driving license to drive within the UK.
  • Relevant post-registration experience and community experience desirable.
  • Access to a vehicle for work purposes.
  • Ability to assess, deliver, and evaluate quality care.
  • Effective communicator.
