About the Role

We are seeking a highly motivated and enthusiastic Registered Nurse to join our innovative community nursing team at Leatherhead Primary Care Network. As a Community Nurse, you will be responsible for providing high-quality care to patients in their own homes, working closely with our multidisciplinary team to deliver person-centered care.

Key Responsibilities

  • Assess and deliver care to patients in their own homes, working closely with our multidisciplinary team
  • Develop and implement holistic care plans, taking into account the individual needs and preferences of each patient
  • Work collaboratively with other healthcare professionals to ensure seamless care delivery
  • Contribute to the development and implementation of innovative practice, improving patient outcomes and experiences
