About the Role

We are seeking a highly motivated and enthusiastic Registered Nurse to join our innovative community nursing team at Leatherhead Primary Care Network. As a Community Nurse, you will be responsible for providing high-quality care to patients in their own homes, working closely with the community hospitals and specialist teams.

Key Responsibilities

  • Assess and deliver care to patients in their own homes
  • Work closely with the community hospitals and specialist teams
  • Provide education and support to patients and their families
  • Contribute to the development of innovative practice and team management
