As a Community Nurse, you will play a vital role in providing high-quality care to patients in their own homes, promoting independence, and improving health outcomes.

Main Responsibilities
  • Assess and deliver holistic care packages to patients in their own homes
  • Work closely with the community hospitals and specialist teams to ensure seamless care transitions
  • Provide ongoing care and support to patients, reducing the risk of admission and hospitalization
  • Deploy the most appropriate staff member(s) in a coordinated and timely way to best use community resources
  • Support people in their homes, care homes, and through in-reach into hospitals where necessary

Person Specification
