Community Nurse

We are seeking a highly motivated and enthusiastic Registered Nurse to join our innovative community nursing team at Epsom and St Helier University Hospitals NHS Trust. As a Community Nurse - Primary Care Network, you will play a vital role in delivering high-quality patient care and contributing to the development of innovative practice within our Primary Care Networks.
Key Responsibilities- Assess, deliver, and evaluate quality care to patients in their homes, care homes, and through in-reach in to hospitals
- Work closely with the community hospitals and specialist teams to ensure a whole system approach to working across Surrey Downs
- Contribute to team management and the development of innovative practice within the Primary Care Networks
- Develop, implement, and evaluate holistic care packages for patients
- Registered Adult Nurse with NMC
- Valid Driving Licence (to drive within UK)
- Relevant post-registration experience and community experience
- Ability to assess, deliver, and evaluate quality care
- Effective communicator
