Key Responsibilities- Assess and manage patient care needs in the community, working closely with multidisciplinary teams to ensure seamless care delivery.
- Develop and implement effective care plans, taking into account individual patient needs and preferences.
- Provide education and support to patients and their families, promoting health and wellbeing in the community.
- Collaborate with healthcare professionals to ensure continuity of care and optimal patient outcomes.
- Contribute to the development and implementation of healthcare policies and procedures, ensuring best practice and patient safety.
- Registered Adult Nurse with NMC registration.
- Valid driving license and access to a vehicle for work purposes.
- Teaching qualification and evidence of continued professional development.
- Experience of team management and multidisciplinary working.
- Relevant post-registration experience in a community healthcare setting.
