About the Role

We are seeking a highly skilled and experienced Community Health Specialist to join our team at Epsom and St Helier University Hospitals NHS Trust. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support in the community.

Key Responsibilities
  • Assess and manage patient care needs in the community, working closely with multidisciplinary teams to ensure seamless care delivery.
  • Develop and implement effective care plans, taking into account individual patient needs and preferences.
  • Provide education and support to patients and their families, promoting health and wellbeing in the community.
  • Collaborate with healthcare professionals to ensure continuity of care and optimal patient outcomes.
  • Contribute to the development and implementation of healthcare policies and procedures, ensuring best practice and patient safety.
Requirements
  • Registered Adult Nurse with NMC registration.
  • Valid driving license and access to a vehicle for work purposes.
  • Teaching qualification and evidence of continued professional development.

  • Relevant post-registration experience in a community healthcare setting.
