Job Summary

We are seeking a highly skilled and dedicated Community Nurse to join our team at Epsom and St Helier University Hospitals NHS Trust. As a Community Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their own homes and communities.

About the Role

The successful candidate will be responsible for assessing the needs of patients and developing personalized care plans to meet their unique requirements. You will work closely with other healthcare professionals to ensure seamless care coordination and delivery.

Key responsibilities will include:

  • Conducting comprehensive assessments of patients' physical and emotional needs
  • Developing and implementing individualized care plans to promote optimal health outcomes
  • Providing education and support to patients and their families to empower them to manage their health effectively
  • Collaborating with other healthcare professionals to ensure comprehensive care coordination
  • Participating in multidisciplinary team meetings to discuss patient care and develop strategies for improvement
Requirements

To be successful in this role, you will need:

  • A valid nursing registration with the NMC
  • A teaching qualification and experience of teaching and training others
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
  • A strong understanding of community health and the ability to work in a fast-paced, dynamic environment
  • Access to a vehicle for work purposes and a valid driving license
