Job Overview

We are seeking a highly motivated and enthusiastic Registered Nurse to join our innovative community nursing team at Leatherhead Primary Care Network. As a Community Health Specialist, you will play a vital role in delivering high-quality care to patients in their own homes, community hospitals, and through in-reach into hospitals.

Main Responsibilities
  • Assess and deliver holistic care packages to patients in the community
  • Work closely with the community hospitals and specialist teams to ensure a whole system approach to care
  • Deploy the most appropriate staff member(s) in a coordinated and timely way to best use community resources
  • Reduce risk of admission for patients and support people in their homes, care homes, and through in-reach into hospitals

Person Specification

We are looking for a Registered Adult Nurse with NMC registration and a valid driving licence to drive within the UK. Relevant post-registration experience and community experience are desirable. You must have access to a vehicle for work purposes and be able to assess, deliver, and evaluate quality care. Effective communication skills are essential.

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020, and a submission for Disclosure to the Disclosure and Barring Service will be required.
